Ordering Instructions

Using the numeric popup menus provided on each page, select the number of each title you need. Print the form and calculate the total and write it in the blank provided. Enter the personal information required (name, address & phone) in the fields provided.

Fax the form(s) to 801-226-2717. The order will be confirmed using the phone number you provide. After confirmation is made, you will be required to send a check for the amount of the sale. The packets will be sent to you immediately.
